  1.  This C.A.I Application/Check List form
  2.  $50 first year's membership dues, payable to the N.C.C.A
  3.  Three (3) letters of personal reference
  4.  Personal resume
  5.  Student/Candidate photo
  6.  Statement as to why student/candidate wants to become 
      certified as a member of the N.C.C.A.
  7.  Photocopy of ministerial credentials* (For Professional 
      Clinical Membership)
  8.  Verification Form from Testing/Measurements course and 
      photo copy of cover page of Student's Personal T.A.P.
  9.  Transcript from your C.A.I.
 10.  Photocopy of highest diploma or degree earned (minimum 
      high school or equivalent required) or official transcript(s)
 11.  Photocopy of cover page from each T.A.P. processed.
         (minimum 10 required for certification/license)
         (minimum 25 required for certification/license as a 
          Pastoral counselor)
 12.  Answers to the fifty (50) Board questions
 13.  Verification Forms from their Clinical Supervisor 
      confirming that Phase I (for certification/license) plus 
      Phase II (for Pastoral counselor) supervision is 
      completed and satisfactory. 
 14.  This canidate does not hold ministerial credentials and 
      desires to complete the course through the N.C.C.C. for a 
      fee of $364.
